Pre-Event Questionnaire Name Email Event Name Address of the event and any specific entry or placement instructions (specific lots, entry ways, loading bays, streets, etc.) Any curbs, ground weight limits, potential clearance issues, grass to stay away from, or any other hazards that we should be aware of when driving in and placing the unit. Name and cell phone number of the contact on-site for day of show contact. Preferred date and time for us to arrive to load-in and setup our unit. Please keep in mind that on average it takes us 30 minutes setup our units. Please allow for an additional hour for DISH Network setup, and 2 hours for camera setup and additional cable runs. Actual date(s) and time that the screen will be running, and daily event hours (if different). Preferred load-out day and time. Parking availability for our pickup truck (if using one of our LED trailers) If we will NOT be using our onboard generator, please advise on power availability. Including where it will be coming from, what type of connection, and if we need to bring any additional cabling.
